VENDOR REGISTRATION APPLICATION
Garki Hospital Abuja (“GHA”) wishes to announce the registration of existing and prospecting vendors wishing to do business or continuing doing business with GHA to register online at www.garkihospital.com under the “Vendors & Businesses” tab. All current and proposed GHA vendors/contractors are required to register in Supplier Portal, an electronic procurement solicitation system, to be notified of upcoming opportunities to do business with GHA.

Current and proposed vendors must complete a Vendor Registration Application form prior to receiving an award or renewing a contract with GHA. The Vendor Registration Application allows vendors/contractors to submit a single application when establishing a contract with GHA for the provision of goods and/or services. A Vendor Registration Form will not be processed for vendors/contractors who have not already registered in the Supplier Portal.
This registration application form must include:
- Letter of introduction by the vendor on a letter headed paper
- Evidence of Corporate Affairs Commission registration
- Evidence of current pharmaceutical license and permission to practice in Nigeria
- Evidence of current tax certificate
- Current price list of drugs
- Evidence of relevant experience and past performance;
- Physical Presence of Business premises
- Evidence of financial capability;
- Registration with any other government regulators; if required
- 1 Passport Photograph of the Manager / Director
- A proof of non-refundable registration fee of =N=20,000/=N=50,000/=N=100,000(As appropriate)
